Would you like to be part of an integrated Discharge Team?

We are looking for an enthusiastic, pro-active individual to join our team. The Discharge Team will provide you with the opportunity to work within an inter-disciplinary team, completing holistic assessments, liaising with ward therapists, Dr’s and the nursing team to coordinate safe, timely discharges. The team work collaboratively with specialist services both within the hospital and in the community to ensure the patients needs are met within the most appropriate environment.

You will need excellent communication and interpersonal skills and have the ability to case manage complex patients, whilst supporting junior staff. You will develop your knowledge and skills across multiple professions and complete assessments on behalf of community services.
