A high profile role as a Critical Care Outreach Practitioner - In one of the largest NHS teaching trusts in the country with over 23 specialities including Level 1 major trauma centre. UHCW is a provider of innovative and leading edge clinical services to Coventry and Warwickshire.

The Outreach service currently covers over 1200 adult inpatient beds 7 days a week 07:30-20:00 going to a 24/7 service in the future. The Outreach role is involved in the early detection and management of the acutely ill deteriorating patient, facilitation of the smooth transition of patients stepping down from critical care and the provision of education and training for the multidisciplinary team.

The Outreach team lead the management of weaning and decannulation of all acute Tracheostomised patients discharged from Critical Care to the general wards including Neurosurgical Patients.

The Outreach team deliver a monthly rolling programme of Tracheostomy and suction training Trust wide. The Outreach team are facilitators on the AIM and ALERT courses and are part of the foundation doctors teaching programmes.
The Outreach team is proactive in responding to patients triggering a NEWS alert when monitoring the VitalPAC electronic observation system.
